Lunch and Learn Webinar: Roadmap to ACO Participation 


This educational session has been developed by Caravan Health to provide rural health care professionals opportunities to learn more about requirements and benefits of Medicare Shared Savings Program ACO participation.


According to Lynn Barr, CEO of Caravan Health, “Managing population health is great for the health and well-being of rural communities. Furthermore, the hundreds of rural hospitals that are embracing ACO’s and population health are finding it results in greater patient loyalty, greater market share and enhanced financial performance.”


PRESENTER: Kathy Whitmire, Vice President of Business Development for Caravan Health and Caravan's sister company, National Rural Accountable Care Consortium, a 501(c)3 not-for-profit organization formed to provide population health infrastructure, education and resources so that independent physician practices and community health systems can participate in new value-based alternative payment models (APM). Responsibilities include business development and oversight of continuous quality improvement for Caravan Health and the NRACC Practice Transformation Network (PTN).
